Announces Plans for Direct Public Offering

SAN DIEGO, Calif. – July 20, 2015 – Global Transitional Care LLC, the first Medicare approved dedicated Transitional Care Company in the country (“Global Transitional Care” or the “Company”) today announced that it has engaged Capital Services Group to provide strategic financial advisory services including investment packaging and integration of its Rule 506c Plug-In into its corporate website under a direct public offering of its shares.

The offering will be made available to the general public, its customers and other interested parties, under direct solicitation rules outlined in the new Rule 506(c), which eliminates the prohibition against general solicitation and advertising in connection with offerings under Rules 506 and 144A under the Securities Act of 1933.

Transitional Care Management (TCM) is a new area of healthcare support. TCM is in depth planning and management of post-acute care for certain chronic diseases, providing for more continuity across the continuum of care. TCM begins in the hospital, just prior to discharge, where the patient’s needs are assessed in concert with the physician’s orders and an at-home plan is established. The TCN coordinates the necessary care at home and manages the overall plan of care, in-person, at the patient’s residence.

Fifteen to twenty percent of Medicare patients are re-admitted to the hospital within thirty days of discharge. With Transitional Care Management, patients have a proven success rate in avoiding re-admittance.
